Primary Care Networks (or PCNs) – which bring together groups of local practices – now have a key role to play in enabling proactive, personalised, and integrated health and social care.
12 PCNs are now hard at work in Newcastle and Gateshead, with a focus on preventing ill health and tackling health inequalities, supporting the workforce, maximising data and technology opportunities, and overall efficiency.
By bringing together groups of practices, PCNs are able to take on a wider set of staff roles than individual practices, including areas like physiotherapy and social prescribing.
Our Involvement Team supports the networks to help them engage with and listen to patients at local level.
